- Hazard – The jaws can loosen from the chuck and break off, posing a laceration hazard.
- Remedy -Consumers should immediately stop using the recalled jaw chuck systems and contact Penn State Industries to receive a full refund of the purchase price.
- Incidents/Injuries – Seven reports of the jaw loosening or ejecting from the chuck. No injuries have been reported.
12/6/18 – Skip Hop Tuo convertible high chairs
- Hazard – The legs on the highchair can detach from the seat, posing fall and injury hazards to children.
- Remedy – Consumers should immediately stop using the recalled high chairs and contact Skip Hop for a full refund or an e gift card of equal value.
- Incidents/Injuries – 17 reports of the legs of the high chairs detaching. No injuries have been reported.
12/6/18 – Kawasaki Mule™ Pro off-highway utility vehicles
- Hazard – The vehicle’s rollover protection structure (ROPS) can fail to protect consumers, posing an injury hazard.
- Remedy – Consumers should immediately stop using the recalled utility vehicles and contact a Kawasaki dealer to schedule a free repair. Kawasaki is contacting all known purchasers directly.
- Incidents/Injuries – None Reported
12/6/18 – Meijer Falls Creek Kids infant and toddler hoodies
- Hazard – The rollover protective structure (ROPS) on the mower can loosen and fail to protect the operator in a rollover accident, posing a risk of injury.
- Remedy – Consumers should immediately take the recalled hoodies away from children, stop using them and return them to any Meijer store for a full refund.
- Incidents/Injuries – One report of the zipper slider body and pull detaching from a hoodie. No injuries have been reported.
12/4/18 – Signature Hardware Wall-mounted shower seats
- Hazard – The aluminum hardware supporting the shower seat can corrode, posing fall and laceration hazards.
- Remedy – Consumers should immediately stop using the recalled shower seats and contact Signature Hardware to receive a full refund.
- Incidents/Injuries – 194 reports of the shower seats breaking, including 37 reports of falls without injuries and 23 reports of injuries. Injuries include scrapes and bruises, three reports of lacerations for which consumers received stitches and one report of lumbar fractures.
